To arrange our date, I have prepared a simple booking form to fill in.
Simply start with a hello and warm introduction about yourself and details of our date. If you have any requests, please don't hesitate to bring it up in a respectful manner. The more I know about you, the more it would help to establish our chemistry when we finally meet.
Screening is necessary but kept simple, with a form of ID or a provider reference as a requirement.
Trust goes both ways, and you can be sure that any information provided is held in the strictest confidentiality.
